in addition, perform dermatologic surgery for the treatment of

Patients are seen by appointment, although accommodations will

office procedures and medical equipment have been chosen with
much thought and care to provide quality medical services in a
pleasant, efficient and friendly atmosphere. On
your first visit, a permanent record will be initiated. Your
cooperation in providing the requested data on the patient
registration forms is appreciated. All your records will be
treated confidentially. We will forward consultation reports to
your primary care physician. |

request that patients make all payments, including co-payments,
at the time of service. Cash, check, MasterCard and Visa, are
all accepted. We will submit bills from our Concord and
Cambridge offices to all insurance plans with which we
participate. For a list of insurance plans, click here.
At the time of your visit, you will receive an itemized form
which you can submit to your insurance company if necessary. Please retain this form for future insurance or income tax
